Completion of this form does not confirm a nomination. Rather, this allows OCASA to be in touch with the nominator to discuss the nomination further, and to assist in the successful completion of a nomination. You may make one nomination, or more, and in any category. Nominations can be for any OCASA member in the system. If youíre not sure of the OCASA membership status, include their name here and weíll confirm. Submissions should be received by March 15.
